Early Intervention Intake & Transition Specialist

Ensures thorough and timely intake of new families (ages 0-5). Initiates and participates in the ongoing planning and delivery of Early Intervention services (ages 0-2), including family counseling, intake, assessment, and placement, into Board services (ages 0-5). Provides families with ongoing assistance in accessing Board and community resources. Team Leader and facilitator of weekly regional meetings designed to match appropriate team to complete eligibility determination for Part C service(s).  Creates agendas for meetings that also allow time for weekly teaming and coaching opportunities to support enrolled families. Obtains pertinent information regarding children’s medical and family background in preparation for weekly teaming meetings. Participates in individual programming planning and Individual Family Service Plans (IFSP) meetings to explain Board service and supports in an individualized manner. Act in compliance with federal, state, and agency rules and regulations through the teaming process. Maintains working relationship with local school districts and other community agencies to coordinate Board services. Facilitates transition from the Early Intervention program to other pre-K programs. Provide quality assurance of IFSP. Maintains efficient and accurate case records and documentation of services provided to families in agency database, completes forms as required.
